Working remotely from New York, you’ll play a crucial role in supporting our members' healthcare journeys. In this position, you’ll interact with a diverse array of individuals, performing member outreach either over the phone or through home visits. Your goal will be to engage members, discuss their care plans, and ensure they have access to the necessary resources and education throughout their healthcare experience. Collaboration is key; you'll coordinate with healthcare providers, community partners, and caregivers to facilitate personalized care and address any inquiries or concerns. With 1 to 2 years of relevant experience and a high school diploma or GED, you'll be part of a dedicated team focused on continuity of care and member satisfaction. You'll also document member needs, maintain compliance with regulatory guidelines, and educate members about their benefits.
