Intake and Discharge Coordinator - 10439

The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.


The Intake/Discharge Coordinator facilitates the intake, discharge, and navigation of participants in our Recuperative Care programs. This position reports to the SSRCC Operations Manager.

Essential Job Functions 
  • Acts as the point person for the day-to day operations of Recuperative Care Intake and Discharge System; ensures smooth coordination of daily intake process  receiving programs and property management.
  • Understands contractual requirements and works with Operations Manager to ensure Intake/Discharge Operations are meeting all targets.
  • Identifies goals and strategies to better conduct integrated-care procedures, better serving the needs of homeless individuals.
  • Provides ongoing program and navigation services in conjunction with integrated care team.  
  • Performs other duties as assigned.


Qualifications Summary 
  • Sound critical thinking and decision-making skills.
  • Knowledge of homeless issues and demonstrated sensitivity to and a desire to work with under served populations.
  • Ability to take initiative to implement projects and pursue goals with minimal supervision.
Education and Work Experience
  • Must have a valid driver's license.
  • Microsoft Excel or other spreadsheet program experience is a requirement.
  • Bachelor’s degree or relevant experience in a related field is preferred.        
  • Experience serving homeless community or public health sector preferred.  
  • Excellent organization skills and willingness to work flexibly under pressure.


$23.20 - $27.80 an hour
