We are seeking a highly motivated Outreach Coordinator to join our team in the Bronx. This hybrid role focuses on making outreach calls to clients receiving care management services and promoting additional offerings, such as Early Intervention (EI) and Home and Community-Based Services (HCBS). The ideal candidate will possess strong communication and customer service skills, with the ability to effectively engage clients and increase service enrollments.
*Key Responsibilities:*
• Conduct outreach through phone calls, emails, and follow-up communications to clients on a designated roster.
• Provide thorough information about available services, including Early Intervention (EI), HCBS, and other programs.
• Identify clients’ potential needs and align them with appropriate services.
• Proactively present and promote additional services and benefits to existing clients.
• Collaborate with care managers to ensure clients understand how additional services can improve their outcomes.
• Address questions and overcome objections by educating clients on the long-term value of offered services.
• Maintain accurate and detailed records of outreach efforts and outcomes in the system.
• Track enrollments and provide feedback to management on outreach effectiveness.
• Prepare periodic reports on client engagement and service enrollment growth.
• Work closely with care managers, service providers, and other team members to ensure smooth transitions for clients enrolling in additional programs.
• Provide updates on progress and contribute to team efforts aimed at improving service delivery.
*Qualifications:*
• Bachelor’s degree in social work, healthcare management, communications, or a related field (preferred but not required).
• 1-3 years of experience in outreach, client services, or sales in healthcare or social services.
• Strong communication, interpersonal, and persuasion skills.
• Ability to manage multiple outreach campaigns and meet enrollment targets.
• Proficiency in Microsoft Office and CRM tools for tracking outreach activities.
• Knowledge of Early Intervention (EI), HCBS, and other social service programs (preferred).
