Mobile Dietitian

Job Responsibilities : Conducting timely nutrition assessments based on clinical and subclinical information including physical assessment  and laboratory values, completing Care Area Assessments, developing and updating Care Plans, and recommending appropriate nutrition intervention to maintain or improve nutrition status utilizing clinical software applications to facilitate care Monitoring and documenting on monthly/weekly/daily weights Completing timely and accurate MDS Participating in the multidisciplinary team, quality of life, and family care conferences to discuss resident nutrition status and/or implement or change diet order, enteral or parenteral feeding Providing nutrition education to discharging residents/families Communicating and collaborating with the food service department Education and Experience Requirements: Maintains current license to practice dietetics in Ohio (LD) and active certification as a Registered Dietitian (RD) ServSafe Level 2 Certification preferred but not required Strong critical thinking skills Why Legacy Health Services?
