Position Overview:

We are currently seeking a skilled and experienced Home Health RN Case Manager to join our team in Carson City. As an RN Case Manager, you will play a crucial role in coordinating and providing comprehensive home health services to our patients.



This is a Full-Time position with a competitive compensation package Starting at $100,000 per hour, based on a Pay Per Visit Model.


Responsibilities:
  • Conduct initial assessments and develop individualized care plans for patients.
  • Coordinate and provide skilled nursing care in a home setting.
  • Collaborate with interdisciplinary healthcare teams to ensure the best possible patient outcomes.
  • Educate patients and their families on healthcare management and promote self-care.
  • Monitor and evaluate patients' progress and adjust care plans as needed.


Qualifications:
  • Current and valid Registered Nurse (RN) license in the state of Nevada.
  • Minimum of 1 year of experience in home health or a related field.
  • Strong clinical and assessment skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a collaborative team.
