As a Home Health Physical Therapist at Phoenix Home Care & Hospice, you will provide physical therapy services to patients in their homes, following a physician's prescribed plan of care. You will engage in case conferences to ensure quality patient care, apply sound judgment and technical skills while adhering to agency protocols, maintain compliance with ethical standards, and routinely practice safety measures. You might also perform additional job-related duties assigned by your supervisor.

To qualify for the Home Health Physical Therapist position at Phoenix Home Care & Hospice, candidates must hold an active physical therapy license in the state of MO. While home health experience is preferred, it is not a strict requirement.
