Practice Facilitator

 

As a Practice Facilitator, you will assist a group of primary care physician offices in improving the care delivered to HMP members, increase compliance with clinical best practices, and build the capacity to support an ongoing quality improvement program in their practice setting.  Specific tasks would include assessment of clinic processes, identifying gaps in health care provider performance, providing monthly education or academic detailing to providers, and providing periodic performance reports. PF will also pitch our program to new clinics.  

 

Hours: 8-4:30 Monday- Friday CST

 

In this role, you will:

Assess practice processes, including workflows, quality improvement, and review of claims data and clinical records to identify opportunities for improvement.

Identify gaps in health care provider performance using practice-level data and recommend improvement methods and processes.

Assist in the implementation of the methods and processes.

Provide education on improvement methods and processes to providers and other practice personnel. 

Assist in the implementation of the methods and processes.

Provide periodic performance measurement reports to the practice, documenting clinical performance on a set of standardized quality measures.

Develop and maintain activity and/or project plans.

Ensures plans, tasks, and timelines meet contractual and Company requirements and guidelines.

Provides regular updates to Company management on activity and/or project status.


What You'll Need:

Current Oklahoma RN without Restrictions.

5+ years of experience in healthcare and/or equivalent training and/or experience.

3+ years experience in an ambulatory clinic setting.

Disease management experience.

Strong communication skills.

Good documentation and writing skills.

Must have a valid driver's license and be willing to travel in Oklahoma.

Experience in a primary care setting is preferred.

Experience with quality improvement initiatives preferred.
