Company Description 

The mission of Oak Street Health is to rebuild healthcare as it should be.  

Oak Street Health and the Risk Bearing Entity (RBE) are seeking to hire an EMR Access Coordinator to facilitate access for teams within the RBE. This newly created role will involve collaborating with various teams across the RBE to set up Electronic Medical Record (EMR) access for existing and new team members. The coordinator will address access gaps, communicate with teams and leaders regarding access issues, and work alongside Core OSH EMR access coordinators and population health leaders to implement OSH at new facilities. As the RBE expands in 2025, this position will be central to the success of the RBE teams at OSH.

Role Description: 

The EMR Access Coordinator ensures that our care teams have access to their patients' complete medical histories by assisting each member in obtaining and maintaining access to the Electronic Medical Records (EMR) systems for hospitals and other healthcare facilities where their patients receive care. The EMR Access Coordinator will help care teams submit access requests to hospitals, follow up to ensure access is granted in a timely fashion, track EMR access for each team member, and collaborate with regional leaders to troubleshoot any issues that may arise.

This is a remote role.

Core Responsibilities: 

• Submit EMR access requests on behalf of care team members via email, fax, or mail as needed.  

• Follow up with hospitals by phone or email regarding submitted requests to ensure timely access.  

• Track expiration dates for EMR access accounts and renew them as necessary.  

• Close out EMR access accounts when care team members leave their roles.  

• Assist care teams in troubleshooting EMR access issues and facilitate communication with outside hospitals or facilities.  

• Maintain the master RBE access tracker to document where each RBE team member has EMR access.  

• Regularly send out summary statistics based on the master RBE access tracker.  

• Perform other duties as assigned.

Pay Range

The typical pay range for this role is:

$17.00 - $31.30

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
