As a Phoenix Home Health LPN, you will be the point of contact between a patient and medical providers. The LPN should possess strong skills in physical assessment, and communication to complete the individualized plan of care according to physician orders that incorporates the analysis of initial assessment. Working alongside other nurses, Physical Therapist, Speech Therapist and or Occupational Therapist.

In this full-time role, you'll work Monday to Friday from 8 am to 5 pm, with some occasional weekend rotations. Based around St. Louis, MO, this position not only comes with a competitive pay range of $58,000 to $72,000, but also an enticing sign-on bonus. Your primary responsibility will involve being the pivotal point of contact between our dedicated patients and their medical providers. You'll utilize your strong assessment and communication skills to create personalized care plans based on physician orders. Working alongside other healthcare professionals, including nurses and various therapists, you will ensure that our clients receive the best possible care.
