We are seeking a dedicated and compassionate Registered Nurse to join our Home Health team. Our Home Health RNs should possess strong skills in physical assessment, diagnostic interpretation, and communication to formulate an individualized plan of care according to physician orders that incorporates the analysis of initial assessment. As a Registered Nurse, you will play a crucial role in providing high-quality medical care, supporting patients through their recovery processes, and collaborating with other healthcare professionals to ensure optimal patient outcomes.

Duties

  • Assess and monitor patient health conditions, including vital signs and symptoms. 
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans. 
  • Provide education and support to patients and their families regarding health management and treatment options. 
  • Maintain accurate patient records and documentation in compliance with healthcare regulations. 
  • Ensure adherence to infection control protocols and sterile processing standards. 
  • Participates in education programs, quality assessment activities, nurse’s meetings and other staff meetings as required.

  • Capable of conducting a comprehensive assessment of a patient's condition, taking into account their medical history, physical well-being, and home surroundings.
  • Execute a range of nursing procedures, including wound care, medication administration, and equipment management, with a focus on safety and effectiveness.
  • Possess a thorough understanding of various diseases, medical conditions, and treatment options to guarantee safe and accurate administration of medications.
  • Strong critical thinking skills for effective decision-making in high-pressure situations.
  • Excellent communication skills for educating patients and collaborating with healthcare teams. 
  • Demonstrate a capacity to adapt to various and evolving home settings and patient requirements, showcasing flexibility and ingenuity.
  • Exhibit empathy, patience, and kindness in your interactions with patients and their families, fostering trust and developing strong connections.

Must Haves

  • Active Registered Nurse License in the state of MO
  • Home Health experience required.
  • Driver's License and Vehicle with Current Auto Insurance
