Responsibilities

Collaborate with Primary Care Providers and Medical Directors
Triage inbound communications from teammates and patients
Engage in comprehensive chart preparation
Oversee post-appointment processes
Conduct virtual patient assessments and develop care plans
Promote patient health through education on preventive care
Participate in crisis intervention discussions

Education

Current unrestricted LVN/LPN license
2 years of clinical experience preferred
