SFDPH strives to achieve its mission through the work of multiple divisions - the San Francisco Health Network, Population Health, Behavioral Health Services, and Administration. The San Francisco Health Network is the City’s only complete system of care and has locations throughout the City, including Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG), Laguna Honda Hospital and Rehabilitation Center (LHH), and over 15 primary care health centers. The Population Health Division (PHD) provides core public health services for the City and County of San Francisco: health protection, health promotion, disease and injury prevention, and disaster preparedness and response. Behavioral Health Services operates in conjunction with SFHN and provides a range of mental health and substance use treatment services.

Job Description

Under the direction of the Chief Executive Officer/Nursing Home Administrator, the primary goal and responsibility of the Assistant Nursing Home Administrator (ANHA), Support Services (0941 Manager VI) is to provide a home-like environment for residents of the distinct part skilled nursing facility (D/P SNF) at the Laguna Honda Hospital and Rehabilitation Center (LHH) and to protect and promote their resident rights. This position leads, guides, and directs the operations of the D/P SNF healthcare facility in accordance with local, state, and federal regulations, standards, and established facility policies and procedures to provide appropriate care and services to residents.

The Assistant Nursing Home Administrator (ANHA), Support Services (0941 Manager VI) provides leadership and direction in developing new programs and establishing organizational policies, priorities, and objectives, and directs and coordinates the implementation of program changes. The Assistant Nursing Home Administrator (ANHA), Support Services (0941 Manager VI) makes decisions that directly impact the facility including the health and welfare of the residents cared for, staff employed, and the community served.

The Assistant Nursing Home Administrator (ANHA), Support Services (0941 Manager VI), Support Services performs the following essential job functions:

  • Plans, develops, organizes, implements, evaluates, and directs the overall operations of the Environmental Services, Food and Nutrition Services, Clinical Nutrition, Security and Materials Management and Biomedical Engineering divisions of the facility and its programs and activities, in accordance with current state and federal laws, regulations, and guidelines issued by the governing body.
  • In collaboration with leadership team, identifies and evaluates the facility’s key performance indicators and outcomes. Establishes an ongoing system to monitor these key indicators and creates effective action plans via the Quality Assurance and Performance Improvement process.
  • Serves as subject matter expert and makes critical decisions on general skilled nursing facility practices and procedures, Omnibus Budget Reconciliation Act (OBRA) regulations, Code of Federal Regulations (CFR), State Regulations (Title 22), Appendix PP State Operations Manual, Life Safety Code regulations, applicable labor relations laws, and all other regulatory entities that may apply.
  • In collaboration with the Chief Financial Officer, develops and implements a census and occupancy management strategy and other processes/systems to improve census and maintain the fiscal health of the facility. Co-leads weekly or bi-monthly budget compliance meetings to ensure financial goals are met.
  • Develops and maintains positive relationships with residents, employees, and family members through scheduled, deliberate positive interactions with them using face to face, phone, or other means of communication. Promotes and encourages an environment of trust among all employees related to the overarching goal of resident safety and abuse prevention.
  • Collaborates with Human Resources on recruiting and hiring qualified individuals to fill vacant positions and maintain a strong and capable team.
  • Makes written and oral reports/recommendations to the governing board concerning the operation of the facility. Reviews and interprets monthly financial statements and provide relevant information to the governing board.
  • Prepares, guides, and directs facility readiness for respective divisions related to health inspection surveys, Occupational Safety and Health Administration (OSHA) surveys, and any other regulatory entity or requirement. Oversees addressing results of mock surveys or similar processes used to identify issues in preparation for health surveys, OSHA surveys, and any other regulatory requirements.
  • Ensures the facility’s plan of correction response to any regulatory, inspection survey is completed, adequate, implemented, and timely for each supervised division.
  • Ensures resident incidents and concerns that rise to a reportable event such as alleged abuse, neglect, mistreatment, misappropriation, etc. are reported to the correct entity within the stated regulatory requirement.
  • Promotes safe work practices, safety rules, and accident prevention procedures to prevent employee injury and illness. Manages and minimizes facility risk through a team approach to achieve desired outcomes in customer service, key performance indicators, employee retention, and other areas as identified.
  • Continues to learn and teach others about new Centers for Medicare & Medicaid Services (CMS) program initiatives such as value-based purchasing programs like SNF-VBP, SNF-QRP, Quality Measures, Nursing Home Compare, Payroll Based Journal reporting, etc. Stays informed regarding regulatory, business practices, standards of care and other changes influencing facility outcomes; thereby, facilitating continued success for all.

The Assistant Nursing Home Administrator (ANHA), Support Services (0941 Manager VI) may perform other duties as required or assigned.

Qualifications

REQUIRED MINIMUM QUALIFICATIONS

  • License: Possession of a current, valid, unrestricted license as a Nursing Home Administrator issued by the California Department of Public Health, OR eligibility to register for and pass the Nursing Home Administrator State and National Examinations per regulations (see Health and Safety Code Section 1416.55(d)) within twelve months of hire, AND
  • Experience: Five (5) years of verifiable, full-time experience managing in a long-term care environment, all of which must include supervision of professionals.

Applicants must meet the minimum qualification requirement by the final filing date unless otherwise noted. 

One-year fulltime employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40-hour work week).

DESIRABLE QUALIFICATIONS

The stated desirable qualifications may be used to identify job finalists at the end of the selection process when candidates are referred for hiring. 

  • Active membership in the California Association of Health Facilities (CAHF) and/or American College of Healthcare Executives (ACHE)
  • Completion of ACHE or CAHF Leadership Training or equivalent training
  • Success in contributing to the certification or recertification in a management role within a Long-term Care facility
  • Successful management and leadership experience in a licensed long-term care facility managed by federal (such as VA), state, or local government
