We are seeking a compassionate and highly organized Family Support Service Liaison to join our team in Monsey. This full-time role involves working closely with individuals and families receiving support services, helping them navigate available resources, coordinating care, and ensuring families feel heard, supported, and empowered.
*Key Responsibilities:*
Serve as the primary point of contact for families accessing support services.
Assist families in understanding and utilizing available resources, including OPWDD services.
Provide referrals and help coordinate services such as respite, community habilitation, and parent training.
Maintain regular communication with families to monitor needs and progress.
Advocate on behalf of families with service providers and agencies.
Collaborate with internal teams to ensure service plans align with family goals.
Maintain accurate records and documentation in compliance with agency and regulatory standards.
Attend community meetings and events to stay informed and build connections.
*Qualifications:*
Bachelor’s degree in social work, psychology, human services, or related field preferred.
Experience working with individuals with developmental disabilities or similar populations.
Strong interpersonal and communication skills.
Yiddish-speaking strongly preferred, due to the needs of the local community.
Knowledge of OPWDD services and family support programs a plus.
Ability to work independently and as part of a collaborative team.
Organized, detail-oriented, and empathetic.
