Position: Registered Nurse Case Manager

What We’re Looking For:
Registered Nurse (RN) License in the state of California
Home Health Experience: Minimum of one (1) year under your RN license
CPR Certification: Must be current and valid
Driver’s License & Insured Vehicle: Required for home visits
Home Health experience Preferred
Key Responsibilities:
Comprehensive Patient Assessment: Conduct initial assessments of patients and their families to identify home care needs, including a full physical examination and a thorough history of current and past illnesses.
Ongoing Evaluation: Regularly reassess patients' nursing needs and update care plans as necessary.
Care Plan Development: Create, implement, and revise individualized care plans based on nursing diagnoses, incorporating therapeutic, preventive, and rehabilitative actions. Engage patients and their families in the planning process.
Health Assessments: Utilize health assessment data to accurately determine nursing diagnoses.
Plan of Care: Initiate and manage the patient’s plan of care, ensuring that all revisions are made promptly as patient needs evolve.
