As a vital member of the Interdisciplinary Team (IDT), you will collaborate closely with internal team members, community behavioral health specialists, and partner organizations to deliver comprehensive care. Whether it's conducting psychiatric assessments, supporting care planning, or guiding behavioral health interventions, your expertise will shape participant care and well-being.

What does success look like as a Behavioral Health Nurse Practitioner?
  • Comprehensive Psychiatric Assessments: Conduct in-person or telehealth assessments and collaborate with the IDT to develop personalized care plans.
  • Collaboration with Mental Health Counselors: Partner with the Mental Health Counselor to guide IDT meetings, bridging knowledge gaps on behavioral health issues, and empowering the team with tools for effective interventions.
  • Behavioral Health Education: Educate IDT members on mental health disorders, substance abuse, dementia-related behaviors, and goal setting for meaningful behavioral interventions.
  • Integrating Behavioral Health Care: Work closely with clinicians to integrate behavioral health into participants' care plans seamlessly.
  • Building Community Partnerships: Establish and maintain strong communication with community behavioral health specialists, ensuring collaborative care.
  • Coordination with Geriatric Psychiatrists: Manage schedules and facilitate the exchange of critical information to make the most of geriatric psychiatric consultations.
  • Inpatient Follow-Up: Support participants admitted to psychiatric facilities to ensure issues are addressed, and follow-up care is well-planned.
  • Behavioral Health Diagnosis: Evaluate participants with complex behavioral health issues to develop accurate diagnoses and tailored care plans.
  • Team Training & Support: Provide behavioral health education to team members, equipping them with knowledge to enhance participant care.
  • Contribute to Organizational Growth: Embrace additional opportunities and responsibilities as needed, actively participating in the continued growth and success of the organization.


What does an ideal candidate look like?
  • Passion for serving high-risk seniors and frail older adults.
  • 3+ years of experience providing behavioral healthcare to elderly or complex populations.
  • Experience working with Interdisciplinary Teams in a healthcare setting.
  • Active Psychiatric Mental Health Nurse Practitioner State License without restrictions.
  • State CDS Certification and DEA License.
  • NPI License required.
  • PACE experience preferred but not required.
  • CPR and First Aid certification (or ability to obtain within 60 days).
  • Valid CA driver’s license, personal transportation, good driving record, and auto insurance (as required by state law).


$130,000 - $180,000 a year
