Your responsibilities will include facilitating communication among participants, their families, and our interdisciplinary team (IDT) to guarantee a smooth transition into the PACE program. You'll play an essential part in managing participant satisfaction, assisting with service recovery projects, contacting participants to assess their experience, and advocating for their needs throughout their journey with us. We're looking for someone with a background in healthcare, strong communication skills, and a customer-first attitude. Being bilingual in Spanish is a huge plus!
