Description

  Previous work for Easter Seals TN or Prospect Inc. increases starting pay. 


 Pay is $14.25 per hour.


  1. General Outline of Duties: § Transport service recipients to and from community based outings § Transport service recipients to and from volunteer outings § Transport service recipients to and from employment services § Ensure a safe and pleasant work environment at all times § Dispense daily medication as needed § Model and promote a positive and professional environment § Attend in-service training sessions, meetings, and out of town activities as required § Adopt a proactive stance when handling all issues related to recipient support 


  1. Community Based Outings § Collaborate in scheduling creative, community driven, and age-appropriate outings § Complete all appropriate paperwork 


  1. Administrative Duties § Contribute to monthly reviews regarding CB outing information and details § Contribute to daily notes daily notes regarding CB outing information and details § Contribute to Pre-Planning, ISP and COS meetings as called for regarding CB outing information and details. § Fill out Medical Administration Record (MAR) accurately, timely, and responsibly § Assist co-workers and supervisor with various administrative needs  


  1. Ensure a Person Centered, Professional Work Environment § Maintain a positive work atmosphere by acting and communicating in a manner so that you get along with caregivers, agency staff, people supported, co-workers, direct supports and management § Diligently engage in the use of Person Centered practices and People First language § Ensure all written reports, letters and any other form of communication represents ESTN in a professional manner 


Staff are key in providing what is needed to help an individual learn, keep or improve skills necessary to live in a home, providing supports based on an individual's hopes, dreams, and preferences and individuals are afforded privacy and are treated with dignity and respect, to have access to coordinated services and supports determined by the individual's unique strengths, needs and choices. Staff actively participate in all aspects of planning, implementation monitoring and evaluation of member services and supports.


PHYSICAL REQUIREMENTS: § Ability to lift up to 50 pounds individually and 100 pounds with assistance. § Walking, standing, bending, stooping, reaching, gripping, twisting and turning frequently. § Sitting and kneeling occasionally. § Must be able to speak and hear within normal range. 


JOB CONTEXT: § The Activity and Community Outing Instructor works predominantly in a climate-controlled home setting but may experience temperature variations/extremes when assisting with community outings. 


JOB QUALIFICATIONS: · High School diploma or GED certificate and drivers license with an acceptable. Must be able to pass a background and drug test
