We are currently looking for a Clinical Pharmacist (Remote) that will support patients to promote health. We are looking for a candidate who can focus on promoting health as a positive vitality, beyond just the absence of disease. We need someone that can “look upstream” to consider the complex web of interactions in your patient’s history, physiology to promote health and prevent disease and support managing various medical conditions.
What You Will Do
Lead a patient-centered, pharmacist-led disease management program to optimize care for chronic diseases such as diabetes, COPD, and others, while promoting medication adherence and health education.
Collaborate with healthcare providers and clinical professionals to ensure patients receive the best care
Conduct Medication Therapy Management (MTM) and Comprehensive Medication Review (CMR) as well as other professional tasks in expanded clinical programs
Advise patients on medication use and provide patient education
Analyze patient medication profile for drug interactions, suboptimal treatment regimens, cost effective regimens, etc.
Apply evidence based medication guidelines when creating recommendations to healthcare providers
Follow up with patients and healthcare providers to ensure progress towards patient specific treatment goals
Aid in development of appropriate clinical algorithms, pathways, and call scripts to support staff in delivering services
Assist in coordination of care for patients between healthcare providers and pharmacies
Serves as a resource for drug information for other healthcare providers
Ensures compliance with Federal and State laws and standards of practice which govern the profession of pharmacy
Maintains pharmacy and healthcare records and documentation in accordance with all applicable state and federal laws and regulations
Stay updated on current clinical guidelines and advances in chronic disease management.
Provide education to staff and providers as needed.
What will make you successful here
Doctor of Pharmacy (PharmD) Current pharmacist license in good standing with the State Board of Pharmacy in the State where the candidate is licensed
Clinical experience in Medication Therapy Management
Experience in pharmacist-led Clinical Disease Programs – e.g Diabetes, COPD, CHF, etc
Knowledge of healthcare industry practices and HIPAA regulations
Ability to prioritize tasks and manage multiple projects simultaneously
A motivated self-starter who does not need constant direction or feedback to drive results
Ability to make clinical decisions independently
Flexibility to change and resourceful
Proficient with learning new computer systems, basic IT trouble shooting and using multiple monitors
Excellent verbal and written communication skills.
Excellent organizational skills and attention to detail.
Entrepreneurial spirit, a sense of ownership and comfortable operating in ambiguity
Solution oriented with the ability to think strategically and creatively in decision-making
Strong work ethic to exceed expectations and goals
