Registered Nurse - Street Medicine - 10423

The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.


The Registered Nurse works collaboratively within an Integrated Health Care for the Homeless team.  The RN in this position is responsible for performing both independent and delegated duties in the provision of providing comprehensive health care to all Colorado Coalition for the Homeless (CCH) patients.  


This health outreach role focuses on health outreach within an integrated outreach team, as part of Denver Street Outreach Collaborative focusing on Encampment Response, Healthcare within Micro-Communities, Unsanctioned encampments and Sanctioned Encampments 


NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date.  The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance.  Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions 
  • Provides patient-centered care within an integrated, trauma-informed, culturally-aware, harm-reduction model.
  • Develops rapport, trust, and professional relationships with people experiencing homelessness, focusing on the rough-sleeper population in large encampments.
  • Utilizes motivational interviewing techniques to provide optimum client-centered engagement in care, care coordination, and continuity of services while respecting the autonomy of clients.
  • Works collaboratively, advocates respectfully, communicates effectively, and develops partnerships with community organizations and key stakeholders, including but not limited to Homeless Service Providers, Peer Specialists, Denver Department of Public Health and Environment, Department of Public Health, Parks and Recreation, Denver Police, and other city entities.
  • Ability to perform quick and accurate triage assessments with appropriate recommendations of care as outlined in SSHC nursing protocols via telephone or in-person.
  • Administers medications in a safe, evidence-base manner following written protocols or direct provider order, including but not limited to immunizations, antibiotics, insulin, normal saline, psychiatric medications, Depo-Provera and testosterone injections.
  • Collaborates with other members of the multi-disciplinary health care team and coordinates care with internal programs, such as Housing First, Assertive Community Treatment, Respite, Medication Assisted Treatment.
  • Provides transitional, chronic, and complex care management.
  • Provide nursing services as needed all CCH programs including but not limited to Stout Street Health Center, Health Outreach, , and Hospital-based Respite, Transitional Care Management, West End Health Center, Fort Lyon, Housing First, and SIB.
  • Preceptor to new nurses and nursing students.
  • Transports individuals who are homeless using a personal or organizational vehicle. 


Qualifications Summary 
  • AD, BSN, or MSN in nursing from an accredited college or university.
  • Current licensure from the State of Colorado to practice as a Registered Nurse.
  • Certification in Basic Life Support.
  • Successful completion of the privileging and credentialing process for RNs at the Coalition.
  • Preferred experience in ambulatory, public, mental health or community nursing.
  • Highly preferred experience in, emergency room, urgent care, or critical care. 
  • Bilingual, English-Spanish preferred.  


$31.19 - $37.42 an hour
WHERE A CANDIDATE IS PLACED IN THE COMPENSATION RANGE DEPENDS ON TOTAL RELEVANT YEARS OF EXPERIENCE

Qualified candidates are eligible for the NURSE Corps Loan Repayment Program which offers payment of 60% of unpaid nursing education debt in exchange for a two-year commitment at a NURSE Corps approved site with an option to extend to a third year for an additional 25% of the original balance.
