The Fire Department, City of New York (FDNY), seeks a full-time Case Management Nurse in the Bureau of Health Services. Reporting directly to the BHS SUPERVISOR OF NURSES the successful candidate will maintain a comprehensive case management system for the BHS for Medical Qualification of Firefighter and EMS Candidates, Annual Medicals and BHS Clinic for FDNY incumbents. The candidate will use clinical tools, data and other information to complete assessments of patient needs. Create individualized and comprehensive case management and treatment plans. Determine service-connected coverage eligibility for various treatments and treatment programs. Clearly explain the reason for prescribed treatments and treatment plans to patients. Carry out adept nursing assessment of patient health status. Educate patients on benefits, services and other resources. Communicate with patients, physicians and insurance companies to secure successful outcomes. Document complex medical information in our electronic medical record (EMR) to outline and improve health results and determination of eligibility of candidates. Follow case and quality management processes in compliance with regulatory guidelines and company procedures. Accurately review results from medical tests. Select appropriate outside treatment vendors/providers according to members' needs/illness/injury, can successfully make appointments for member without direct supervision. Make appointments with external vendors as needed to accurately evaluate member or candidates ability to safely perform job to which he/she has applied. Conduct telehealth visits to screen members in accordance with current dpt policies and directives, i.e. with suspected and/or confirmed COVID19. Nurse is able to perform IM, intradermal, and SC injections while following best practices. Ability to perform venipuncture when required, according to dpt policies and best evidence-based practice. Schedule: 5 days/week, apprx 1 weekend every 5 weeks
CASE - MANAGEMENT NURSE (FIRE - 50959
1. A valid New York State license as a Registered Nurse, plus a baccalaureate degree in nursing from an accredited college, and three years of clinical nursing experience; or
2. A valid New York State license as a Registered Nurse, plus an associate degree in nursing from an accredited college, and five years of clinical nursing experience; or
3. Education and/or experience equivalent to '1' or 2’ above. However, all candidates must have a valid New York State license as a Registered Nurse.
Note:
To be assigned to Assignment Level II, candidates must have, in addition to meeting the minimum qualification requirements listed above, at least one additional year of clinical nursing experience.
