As a Wound Care Nurse Practitioner at Theoria Medical, your primary responsibilities will include managing and providing wound care to patients in skilled nursing facilities (SNFs), collaborating with nursing staff, documenting patient encounters in our EHR system, and ensuring a high standard of patient care. You will be actively rounding with the wound care team and will also be available for consultations via phone and secure messaging.
