Position Overview

As a Home Health Case Manager, you will play a key role in collaborating with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload. This is a direct care role, involving travel within Boise, ID and surrounding regions to deliver in-home care from admission to discharge. Your work will ensure that each patient receives the highest standard of care, aligned with their needs and the care plan.

What You Can Expect in This Role

  • Full-time Schedule: Monday to Friday, 8:30 AM - 5:00 PM, with rotating evening on-call and weekend shifts
  • Patient Population:  Pediatrics
  • Manageable Caseloads: Daily visit requirements are below the state and national average
  • Compensation: Paid hourly which includes drive time and mileage reimbursement for traveling within Boise and it's surrounding regions
  • Additional Pay: On-call pay and applicable differential pay
  • Bonuses: Sign-on bonus and relocation assistance available (where applicable)

Qualifications

  • Education: ASN or BSN in Nursing
  • Experience: 2 years of RN experience; Home Health or Acute Care, and/or Pediatric experience preferred
  • Licenses/Certifications: Current RN license in the state of Idaho; Basic Life Support (BLS) certification
  • Other Requirements: Valid driver’s license and proof of auto insurance
